You might also think about the delivery and how and when you’ll give birth.

Some women benefit from making a twin birth plan. Whether or not you’ll be able to give birth vaginally depends on the course of your pregnancy, and how you and your babies are doing.

It also matters how your babies are positioned in your womb, whether or not you’re expecting fraternal or identical twins, that share a placenta, and the culture of the hospital and country you’ll give birth in.

Pregnancy diet, nutrition & prenatal vitamins

It will not affect your milk production or ability to breastfeed later whether or not you experience leaking colostrum during pregnancy. The same applies to the size and shape of your breasts.

If you are bothered by acid reflux and heartburn, you could try natural antacids and heartburn remedies like for instance peppermint and milk. Avoid fatty and spicy foods.

Also, remember taking an iron supplement. This helps with your blood loss when giving birth and also prevents anemia during pregnancy.

Ultrasound at 21 weeks

You should have the anomaly scan this week if you haven’t had it already. This is an ultrasound scan to check that your babies are growing and developing as expected.

The sonographer will examine all organs and take measurements. This is also the scan where it’s possible to look for the sex of your babies. Let the sonographer know whether or not you’d like to know their genders.

Fraternal & identical twins ultrasound

If you’re expecting monochorionic twins, you should be monitored more regularly than women pregnant with dichorionic twins. Monochorionic twins are identical twins that share a placenta.

Dichorionic twins are twins with separate placentas. All fraternal twins have separate placentas. About 1/3 of identical twins have separate placentas as well.

Identical twins that share a placenta are more at risk during pregnancy and birth. They are at risk of conditions like Twin to Twin Transfusion Syndrome and weight inequality due to the shared placenta. They are four times more at risk of low birth weight compared to dichorionic twins.

Women pregnant with dichorionic twins have a higher probability of giving birth vaginally and carrying them to term compared to women pregnant with monochorionic twins.

Baby size & development

When you’re 21 weeks pregnant with twins, they will measure approximately 26,5 centimeters (10.43 inches) from crown to heel. Your baby’s kidneys and urinary tracts are working.

They have the important function of keeping the amniotic fluid volume normal. The babies are drinking the amniotic fluid and the kidneys excrete it again.

Your baby’s sense of touch is still under development. They’re examining their surroundings with their hands and putting fingers and toes in their mouths.

Your babies are receiving information about the world and about themselves through the sense of touch and are having new experiences continuously.

Pregnancy symptoms & belly pictures

Some women suffering from hyperemesis gravidarum– excessive nausea and vomiting – an experience that some of the symptoms improve around this week. Other symptoms you may experience are:

  • Acid reflux and heartburn. That can be caused by your uterus taking up quite a lot of space in your stomach. There is medication you can take to remedy this, but you may also benefit from changing your diet.
  • As your uterus grows, it will stretch the skin on your abdomen which can cause itching. A good, rich cream can relieve this.
  • No need to worry if you feel no movement yet. Not all women feel their babies kicking at 21 weeks pregnant with twins. It depends partly on how the placenta(s) is positioned.
  • You may experience leaking a little colostrum from your breasts. It’s a yellowish or clear fluid and full of antibodies and immunoglobulins. It will be the first milk your babies drink after you’ve given birth. When your milk supply increases, it changes from colostrum to breast milk. This usually happens a few days after birth.

In the second half of your pregnancy, you can measure babies from crown to heel.

This is the equivalent of standing height in older persons. Your babies will be approximately 25,5 cm (10.04 inches) from crown to heel when you’re 20 weeks pregnant with twins. However, most sonographers use a multi-parameter assessment to indirectly determine the growth and well-being of the babies.

Standard parameters are:

  • Biparietal diameter (BPD) – the transverse diameter of the head
  • Head circumference (HC)
  • Abdominal circumference (AC)
  • Femur length (FL) – the length of the long bone in the human thigh.

Pregnancy diet, nutrition & prenatal vitamins

Some women experience that a supplement of magnesium is helpful if they feel very tired and worn out. You can also eat foods high in magnesium. Choose green vegetables, nuts, and whole grains.

Remember to take an iron supplement, which helps with the blood loss when giving birth and prevents anemia.

Ultrasound – anomaly scan

If you haven’t had one already, you’ll have the anomaly scan this week or next week, at the latest. This is an ultrasound scan to check that your babies are growing and developing as expected.

The sonographer will be able to look for conditions like spina bifida, which affects the spinal cord. This is also the scan where the sonographer can look for the sex of the babies.

If you’re pregnant with identical twins who share a placenta, you’re more at risk of pregnancy complications compared to women pregnant with fraternal twins.

You need to make sure that your sonographer knows about conditions like Twin to Twin Transfusion Syndrome. If not, you need to be referred to someone who does.

Baby size & development

When you’re 20 weeks pregnant with twins, the babies are beginning to distinguish between different flavors. This is because your babies are drinking from the amniotic fluid. It tastes differently depending on what you – the mother – has eaten.

Babies prefer sweet, and this is exactly how breast milk and infant formula taste. The innate reflexes – such as sucking the thumb and drinking amniotic fluid – become conscious.

Both are important for babies’ survival after birth.

  • You might begin to notice that your navel is “turning inside out,” because your uterus has grown bigger.
  • If you haven’t felt your babies move yet, you should soon. However, be aware that it depends on how your babies and the placenta(s) are positioned in your uterus. You might not feel an actual kick right away. It may feel more like bubbles in your stomach.
  • If you suffer from hyperemesis gravidarum – excessive nausea and vomiting – the symptoms may not clear up completely until your babies are born. Some women experience that some symptoms improve at around 20 weeks.

If you went full term and delivered at 40 weeks, you’d be halfway through your pregnancy next week.

However, twins aren’t allowed to reach 40 weeks due to the risk of complications. So you may very well be halfway through your pregnancy at 19 weeks pregnant with twins.

Remember to take iron supplements – especially if your iron levels are low. The World Health Organization recommends this as part of antenatal care to reduce the risk of low birth weight, maternal anemia, and iron deficiency.

Your babies are approximately 15 cm (5.91 inches) from head to rump when you’re 19 weeks pregnant with twins.

Your babies are able to clench and open their fists. They can reach their own hands across the chest. They can put their fingers in their mouths.

When you give birth to your children, they’ve already had sensory experiences inside the womb. When they taste, smell, see, feel or hear information, it’s sent to their brains.

Complex connections between brain cells and between different brain areas are formed.

Between weeks 15 and 20 their hearing is developing. Even though the children are well protected inside your uterus, they constantly hear the sound of your beating heart, of your intestines working and when you sneeze, cough or laugh.

You may soon be able to experience movement and feel your babies kicking. You don’t need to worry if you feel no movement yet. Not all women are able to feel their babies move at 19 weeks. Other pregnancy symptoms are:

  • Your ligaments extend more and more every day, so you may experience pain in your groin and side – that’s called round ligament pain.
  • Many women don’t have morning sickness at this point in their pregnancy, but it’s not unusual if you still do. You may also feel nauseous due to constipation or hyperemesis gravidarum. Hyperemesis gravidarum is much worse than normal nausea and vomiting of pregnancy. It’s important that you see your doctor or midwife if you have severe nausea and vomiting.

However, be aware that sometimes the sonographer can’t get a good enough view to tell the genders.

You should take iron supplements to prevent anemia and prepare for the blood loss when giving birth.

Also, if you have children, it can be a good idea to talk to them about your pregnancy – to the extent that they’d like. Let them touch your stomach and chat about the new additions to the family. Borrow or buy books about being a sibling of twins. Don’t pressure your child or children to express their feelings, but be prepared to accommodate their reactions – also if they are negative. If they say or ask you something, and you don’t feel that your answer was sufficient, talk to them again. It’s okay to let them know that you’ve thought more about it – maybe they have too.

Ultrasound scans & pictures

It’s time for your anomaly scan soon. You’ll probably have it any time between 18 and 20+6 weeks. The main purpose of the scan is to check that your twins are developing normally. The sonographer will look in detail at your twins bones, heart, brain, spinal cord, face, kidneys and abdomen. It’s also at this scan that a sonographer can look for the sex of your babies. You won’t get a 3D picture similar to the twin ultrasound picture below. At the anomaly scan you can get or buy a 2D black and white image.

Baby size & development

When you’re 18 weeks pregnant with twins, your babies are approximately 14 cm (5.51 inches) from head to rump. Your babies sebaceous glands are starting to produce sebum – oil on the skin. Together with cells that have sloughed off your babies skin, and shed lanugo hair, this becomes vernix. Vernix has a whitish color and looks a bit like cream cheese. It protects the skin of your babies so the skin doesn’t dry out and also has an effect on body temperature regulation. It works as a lubricant, helping your children through the narrow birth canal. It also helps your babies manage the sudden drop in temperature outside the womb, functioning as an insulating layer.

You’ve probably gained some weight by now, and you might get comments about you being “bigger” than average – but not necessarily. How much weight you’ll gain and how large you’ll look is dependent on many factors such as genetics, how fit you are, eating habits and whether or not you’ve given birth to children before.   • More out of breath and sweat more than usual. This is quite normal and is due to changes in your metabolism and increased blood volume.

You should feel your babies move within the next six weeks. If you’re worried, you should get an ultrasound scan to ensure that both babies are well.

Pregnancy diet, nutrition & prenatal vitamins

You should take iron supplements especially if your doctor or midwife recommends it. Also, stay active. You reduce the risk of backaches, constipation, bloating and swelling if you exercise at least half an hour a day.

Don’t force yourself to go to a gym if you don’t like gyms. Pick an activity you like – at least one you don’t hate. Set the bar low if you’re lacking motivation or energy. Going for a short brisk walk is much better than not going at all.

Other people can most likely tell that you’re pregnant by now. However, you can be 17 weeks pregnant with twins and not showing. It’s rare but possible.

It depends on the shape you’re in, your twin pregnancy weighs gain, your pre-pregnancy weight and whether or not this is your first second-trimester pregnancy. Other symptoms are:

  • Your uterus is growing continuously as your babies grow.
  • Your mucous membranes might swell up due to ramped-up hormones, and you may feel like you have a cold all the time. This can make you snore.

  • You may get a vaginal yeast infection. This is normal during pregnancy due to an altered pH value in the vagina. Itching is the main indicator of vaginal yeast infection. You need to see your doctor if you suspect that you might have an infection.
  • You may get sick more often. The body is less able to fight infection because your immune system is suppressed during pregnancy.

  • Round ligament pain is a common twin pregnancy symptom at 16 weeks pregnant with twins. Your uterus is growing rapidly. Before you got pregnant, your uterus weighed between 50-100 grams. At birth it’ll weigh around 1 kilo. You might feel this growth as pain or tenderness in your abdomen. That is what is referred to as round ligament pain. It’s completely normal and nothing to worry about.
  • You may experience increased pigmentation. Many pregnant women get increased pigmentation which result in lighter brown spots on the skin – especially in the face. This is due to an increased hormone production. To avoid this, you should abstain from sunbathing and wear sunscreen. Sunbathing increases the risk of pigmentation. Most often the spots will disappear or be reduced after birth.

Baby size & development

Your twins are approximately 11,5 cm (4.53 inches) from head to rump when you’re 16 weeks pregnant with twins. If you’re expecting a girl (or two), her ovaries at this stage of pregnancy will move from the abdominal cavity and into the pelvis.

The ovaries will already contain all the eggs a girl will ever get. In a few weeks – if you’re expecting boys – their testicles will start the journey from the pelvis to the scrotum. They have already formed primitive sperm cells.

Your children now have apparent nails. Sometimes they get the hiccups by drinking amniotic fluid. You can’t feel this yet but might later in your pregnancy once your children are bigger.

You may be able to tell the gender of your twins if you get an ultrasound scan at 16 weeks pregnant with twins. The pictures below are of identical di-di twins at 15+1 weeks. Di-di twins have separate sacs and placentas.

About 1/3 of identical twins are di-di twins. All fraternal twins are di-di twins.

The pictures below are of identical di-di twins at 15+1 weeks. It’s the same set of twins as the ultrasound scan pictures in 2D. A regular 2D ultrasound creates a cross-sectional view of your babies.

In a 3D ultrasound, many 2D images are taken from various angles and pieced together to form a three-dimensional image.

It’s possible to be 15 weeks pregnant with twins and not be showing but it isn’t likely. Most women will have gained some weight by now. Have a look at the recommendations for weight gain during a twin pregnancy. Unfortunately, there weren’t enough data to provide sufficient information to develop guidelines for underweight women pregnant with twins. Other symptoms than weight gain and a growing belly is:

  • Your uterus is getting bigger – the tip of your uterus is positioned a few fingers below your navel.
  • You may have heart palpitations. It’s usually normal and due to the increased blood volume you gain during pregnancy. Get a medical exam to be sure.
  • Many women feel better and are not nauseous any longer at this point in their pregnancy, but it’s not uncommon if you’re still feeling sick. You can get medication or try acupuncture. For some, acupuncture can be the best trick to get rid of morning sickness.

Baby size & development

When you’re 15 weeks pregnant with twins, your babies are approximately 10 cm (3.94 inches) from head to rump. The skin is covered by the so-called lanugo hair. It’s very thin, soft, downy hair. It is thought to act to keep the vernix on to the children’s thin skin. The skin will constantly grow thicker layer by layer. The skin is – like every other thing on your children – developing, and the patterns and lines on the skin will soon be visible. Every human being has their own unique patterns and lines in the skin on the fingertips, but also in the palms, feet and toes – even identical twins.
Your children’s eyes are still covered by skin which means that they can’t open them yet. However, they already have tiny eyelashes and eyebrows that grow at a very moderate pace while the skin over the eyes gets ready to divide.

You may be able to detect the gender of your babies if you get an ultrasound scan at 15 weeks pregnant with twins. You might also see one or both of them sucking their thumbs – they’re able to do that now.

Identical twins ultrasound picture

This ultrasound picture is of mo-di twin girls at 14+2 weeks. Being mo-di twins mean that you share a placenta and the outer membrane. The girls have their own separate inner membrane. Only identical twins can be mo-di twins.
